Located on California's Central coast, midway between San Francisco and Los
Angeles, Atascadero State Hospital (ASH) is an all-male, maximum security,
forensic facility serving the entire state of California. ASH opened in 1954
and is a completely self-contained psychiatric hospital constructed within
a security perimeter. ASH has a licensed capacity of 1,239 beds, and employs
over 1,800 staff. Patients are referred to the hospital by Superior Courts,
the Department of Corrections, Board of Prison Terms, and other state hospitals.
As a public sector facility, ASH's mission is linked to public policy. ASH has undergone a number of changes due to shifting public, professional, and legislative attitudes regarding the relationship between crime and mental disorders. The hospital's current mission includes public protection, evaluation, and psychiatric treatment.
"Atascadero State Hospital"Atascadero State Hospital offers unparalleled potential for research of the human personality and provides undergraduate and post-graduate programs for physicians through affiliations with four of America's major universities - the University of California, San Francisco; University of California, Los Angeles; University of Southern California; and Stanford University. The treatment programs at the hospital are designed to provide activities and care to groups of patients who have similar problems and needs. A broad overview of these programs includes focus upon admissions evaluation, community planning, intensive psychiatric care, cognitive restructuring, family interaction skills, self-governing skills, neurobehavioral evaluation and treatment, self-assertiveness, interpersonal communications and social orientation.
Atascadero State Hospital offers a vocational rehabilitation program which provides patients the opportunity to learn an increasing number of work trades and job skills under the direction of trained vocational counselors. Through the broadly based Aztec School curriculum the patient can achieve minimum educational competencies, receive a GED or pursue advanced independent studies. The hospital athletic program offers opportunity for intramural competition in basketball and softball and hosts community-based athletic teams during seasonal sports events. Atascadero State Hospital employs over 1800 people and has a volunteer organization in excess of 190 contributing over 3200 service hours per year. The majority of its human resources are involved in the provision of direct patient care following the Interdisciplinary Treatment Team concept. Members of the Interdisciplinary Team include the psychiatrists, psychologists, social workers, rehabilitation therapists and nursing personnel.
The Long Beach State University School of Nurses Psychiatric Nurse Practitioner Program, American Psychological Association-approved Clinical Psychology Internship Program, and Cuesta College Psychiatric Technician Training Program are operated on-site as well.
ASH is a fully state licensed and Joint Commission on Accreditation of Healthcare Organizations (J.C.A.H.O.) accredited facility. In 1993 and 1996, A.S.H. received "Accreditation with Commendation", the highest level of accreditation awarded by the Joint Commission.
ASH is located in a region that encompasses 100 miles of pristine coastline, a rapidly growing wine industry, agricultural lands, ranches and a carefully preserved rural quality. An unpolluted environment spiced with charm and friendliness, the central coast offers the amenities of an urban area in a serene country setting.
The area is host to five state parks, part of the Los Padres National Forest, four lakes, scenic mountains and snug hills and valleys, each contributing to a wide selection of outdoor recreational activities. Among its many activities, San Luis Obispo County boasts its own Mozart Festival and is host to the Mid-State Fair, one of the largest in the State.
The climate is as varied as the terrain with coastal moderation and inland areas experiencing four distinct seasonal changes. The Vacaville Psychiatric Program was established in 1988 by the Department of Mental Health. It is located in Vacaville,CA, a community of approximately 91,000, nestled between rolling hills and strategically located between the major cities of San Francisco and Sacramento. The program operates a 150-bed licensed Acute Psychiatric inpatient hospital and a licensed 83-bed Intermediate/Day Treatment Program located inside the California Medical Facility. The program provides quality mental health evaluation and treatment to inmate/patients who are referred from prisons throughout the State of California.
Treatment Programs within the Psychiatric Program are as follows:
Acute Program: This program is a 24-hour program consisting of five treatment units. These units are specifically designed to provide a means of ongoing psychiatric assessment and treatment of acutely mentally ill inmate/patients within a highly structured and secure environment. The program operates within a 30-day to 60-day length of stay for CDC referred inmate/patients, and within a "10-day or less" length of stay for CMF emergency referred inmate/patients. The overall goal of the Acute Program is to assess and stabilize symptoms of presenting psychiatric illess. The primary goal of the Acute Program is to identify and provide a successful transition to the most appropriate level of care for the inmate/patient, i.e., Intermediate Treatment Program, the CMF Outpatient Psychiatric Program, Atascadero State Hospital, the general population, or parole. A particular emphasis is placed on diagnosis and evaluation, nursing assessment, rehabilitation therapy assessment, and observation by the entire disciplinary team. Diagnosis and evaluation are ongoing processes that continue throughout the inmate/patient's hospitalization.
Intermediate Treatment Program (ITP): This program is a 24-hour licensed program for inpatient intermediate care. The clinical program design and utilizes a collaborate approach to the treatment of the persistently mentally ill which, in conjunction with medical and psychiatric interventions, emphasizes development of the cognitive, social, and functional skills required for the inmate/patient to maximize potential for success in his post-discharge environment. The ITP further addresses the long-term care needs of the chronically mentally ill inmate. The ITP reserves referrals from the licensed Acute Psychiatric Program, California Medical Facility Outpatient Psychiatric Program, and other state prisons.
Substance abuse education and therapy, medication management training, relaxation training, relapse prevention, stress/anger management, ward government, rehabilitative therapies, and therapeutic community meetings teach invaluable problem solving skills. Pre-Release groups and Pre-Parole planning are available to inmate/patients with these needs.
Metropolitan State Hospital is located in Los Angeles County in the city of Norwalk. The patients served are primarily from Southern California and neighboring counties, and the location provides excellent access to patients’ families and to other mental health facilities with which we interface. The current patient population is 900.
The hospital provides a range of psychiatric treatment in a continuum of care. This continuum includes Acute care, Intermediate /Subacute care, Psychiatric Rehabilitation, and a Skilled Nursing Facility. The programs were developed to respond to the identified inpatient treatment needs of the Counties.
Metropolitan State Hospital is clinically organized on a program management model specializing in legal commitments, programs for Spanish speaking patients, Intensive Treatment and Clinical Research Unit, and Asian/Pacific Treatment Unit. The hospital is unique among State facilities serving the mentally disordered in that it admits a large proportion of acutely ill psychiatric patients resulting in a rapid turnover rate and a shorter length of stay.
The Asian/Pacific Treatment Unit is the only service of its type in the country housed in a state mental hospital. It is a joint effort by the California Department of Mental Health, the Los Angeles County Department of Mental Health and the numerous community agencies which comprise the Asian/Pacific Planning Council. Socialization and rehabilitation of the patient in preparation for reentry into the community form the philosophical basis for treatment.
The Intensive Treatment and Research Unit, which is a male inpatient facility for the chronically persons with mental disabilities, is co-sponsored by Metropolitan State Hospital and the School of Medicine at the University of California, Irvine. The program is a major component of research being conducted by the two institutions with the treatment focus directed toward the chronically refractory and treatment resistant patient.
Metropolitan State Hospital is a major teaching resource in Southern California. In addition to continuing educational programs for its staff, it holds numerous college and university affiliations. Included among these are rotations of psychiatric residents and medical students from the School of Medicine at the University of California, Irvine. The hospital offers a pre- and post-doctoral internship program for psychologist and a fully approved internship program for music therapists. Affiliations with Chapman, Cypress, Golden West, Long Beach City, Mount San Antonio, Rio Hondo Community and California State University, Los Angeles provide training for nurses and psychiatric technicians who receive psychiatric inpatient experience at the hospital.
Napa State Hospital is classified as a low to moderate security facility. The hospital has a bed capacity to treat up to 1,362 patients, on an attractive campus with its treatment units located on 138 acres. The hospital admits individuals who have been referred from their home county under a civil commitment or through the courts on a forensic commitment. All individuals admitted to Napa State Hospital suffer from a major mental illness. The most frequent diagnoses include schizophrenic disorders, mood disorders, anxiety disorders and personality disorders.
Patton State Hospital is currently a forensic hospital with a licensed bed capacity of 1287 for individuals who have been committed by the judicial system for treatment. Currently the hospital is conditionally accredited by the Joint Commission on Accreditation of HealthCare Organizations (JCAHO). The Joint Commission's Board of Commissioner's approved a proposal to conduct all regular accreditation surveys on an unannounced basis beginning January 2006. Joint Commission standards deal with organizational quality of care issues and the safety of the environment in which care is provided. The hospital currently provides psychiatric care and treatment to judicially committed, mentally disordered adult individuals. The Individuals served are assigned to a living unit, which is part of one of the seven Treatment Programs. The hospital is currently going through changes regarding how treatment is provided to the Individuals we serve. Currently treatment is delivered through a centralized approach, where the Individuals served and the staff, from throughout the Hospital, come together to participate in services within Psychosocial Rehabilitation (PSR) Malls. Mall interventions are provided, as much as possible, in the context of real-life functioning and in the rhyme of life of the Individual. Thus, a PSR mall extends beyond the context of a building or place and its services are based on the needs of the Individual, not the needs of the program, the staff members or the institution.
PSR Mall are designed to insure that each Individual receives intensive and individual services to promote his/her increased wellness and ability to thrive in the world. All decisions regarding what is offered through each mall are driven by the needs of the Individuals served. Mall services are provided in an environment that is culturally sensitive and strength based. The concept of recovery shall be the guiding principal and operation framework for the mall system.
Each Individual served has a treatment team that they work with in developing the individual Wellness and Recovery Plan. The teams include a Psychiatrist, Psychologist, Social Worker, Rehabilitation Therapist, Registered Nurse, and Psychiatric Technician.
The new state hospital began construction in the fall of 2001 and is scheduled to be complete in early 2005. The hospital will comprise of 1,148,851 gross square feet (gsf) of floor space. This will include 850,722 gsf for clinical services and programs, 157,041 gsf for support services, 74,379 gsf for administration, and 66,709 gsf for plant operations. This 1500 bed hospital will generate over two thousand jobs, and will be the area’s largest employer.

Hospital North
State Hospital North sits on a hillside that overlooks the Clearwater River
and has the capacity to treat up to fifty adult psychiatric patients. All
of the patients who are treated at SHN are committed through the court system
and are dispositioned to the State Hospital for treatment. The medical treatment
staff at SHN includes 2 full time psychiatrists, one Advanced Nurse Practitioner,
and one half time internist. The other clinical staff includes 6 clinicians
and approximately 60 direct care nursing staff.

Montana State Hospital provides inpatient psychiatric services to adults who have serious mental illnesses requiring intensive treatment and rehabilitation not available in local communities. This is the only publicly operated inpatient psychiatric hospital in the state. The hospital has 174 hospital beds and, in addition, operates 2 transitional care units (15 licensed group home beds) on the hospital campus. The average inpatient census for Montana State Hospital in FY2000 was 159. There were 466 admissions. The Montana State Hospital budget for FY 2001 is $17,608,933.
NEVADA - 2
Nevada Mental Health Institute
It currently provides care for those severely mentally ill adult citizens of Nevada living outside Las Vegas and its surrounding communities. It contains two 37-bed co-educational units that were refurbished in 1998. Eight North is the current point of entry for involuntarily admitted clients. Eight South serves as a step-down unit. A new hospital, available for occupancy in 2001, will contain 80 beds divided into four units. In addition to a separate administration building, the hospital will contain two inpatient buildings, a gymnasium, and a wing for the Psychiatric Emergency Department which will have 10 additional beds dedicated to up to 72-hour observation and stabilization. The hospital grounds will be constructed around a large landscaped internal courtyard, to allow inpatients freer access to the outdoors.

Eastern Oregon Psychiatric Hospital
Forensic Psychiatric Services (FPS)
FPS consists of 334 budgeted hospital-level beds on 10 units, and 100 budgeted residential-level beds on three units. Based on court orders, under Oregon Revised Statutes, FPS conducts psychological and psychiatric evaluations, including determination of an individual's ability to aid and assist their attorney in a trial and determination if a mental disease or defect limits a person's criminal responsibility. These evaluations are done on either an outpatient or inpatient basis, depending on individual needs. FPS provides care and treatment to adults found to be guilty except for insanity and sentenced to supervision by the Psychiatric Security Review Board. FPS also provides treatment to restore fitness to proceed in a trial to persons who were determined not competent to aid an assist their attorney.
Psychiatric Recovery Services (PRS)
PRS consists of 247 budgeted hospital-level beds on nine units, located on both a Salem campus and a Portland campus. Five units serve adult patients who have been civilly committed to the hospital due to serious and persistent mental illness, one unit serves as a medical unit serving patients from all areas of the hospital who have a physical illness or other medical need, one unit serves patients who have suffered brain damage, and two units serve general geriatric patients.
UTAH - 1
The Utah State Hospital (USH) is a 24-hour inpatient psychiatric
facility located on East Center Street in Provo, Utah. The hospital serves
people who experience severe and persistent mental illness. The hospital provides
active psychiatric treatment services for 384 patients. The USH serves all
age groups and covers all geographic area of the state. The USH works with
10 mental health centers as part of their continuum of care. All adult and
pediatric beds are allocated to the mental health centers based on population.
Adult services is comprised of four adult treatment units, respectively northwest,
northeast, southeast, and southwest according to their location in the Lucy
Beth Rampton Building. Each unit provides care for a total of 30 men and women
and utilizes several areas designed for patient comfort and interests.
The Children's Unit serves 22 boys and girls ages 6 to 13 years.
The Adolescent Unit serves 50 youth ages 13 to 18 years.
The Life Habilitation Unit (LHU) is a 37 bed, for men and women, adult psychiatric unit. The focus of the unit is to clinically stabilize the patient and teach the necessary life skills to maintain a quality of life free from psychiatric hospitalization
The Geriatric Unit serves 30 patients and is located on the 2nd floor of the Hyde Building. This unit is also known as the "Hope Unit" since it is the goal of this program to offer hope to patients 60 and older who require special understanding, care, and attention.
The Forensic Unit is a maximum security unit and serves 74 male and female patients. The majority of these patients are ordered to the Hospital by the District Court under the Utah State Criminal Code.
Oregon State Hospital operates under the direction of the Department of Human Services, Health Services, Office of Mental Health and Addiction Services. The hospital is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).
The current capacity is 704 beds, which will be reduced to 681 with the planned closure of an adolescent unit. The hospital employs approximately 1,100 employees. The hospital is funded by both state and federal funds. Patients are billed for their cost of care according to their ability to pay. The 2003-2005 budget was approximately $180 million.

b. Total bed days utilized at WINDS Program (Lander): 2,196 available and
481 used.
c. Total bed days utilized at Freedom Residential House (built on the grounds
of the Wyoming State Hospital for adolescents and opened in March 2000): 2,920
available and 1,477 used.
7. Expansion of deferred admission program on Wyoming State Hospital campus
with 321 admissions being diverted from hospitalization in FY01.
8. Expansion of inpatient acute services for inmates from the Department of
Corrections with eight discharges being completed in FY01.
9. Maintenance of effort in collection of self-generated funds equaling $995,886
in FY01.
