OLYMPIA -- The state Department of Social and Health Services has stopped admissions and revoked the license of Shoquist Adult Family Home, privately operated at 11621 North Fairview Road, Mead.

The home is licensed and owned by Cheryl Shoquist.

The state took the action after finding numerous violations that affected the safety and well-being of residents. The licensee:

• Lacked understanding of the rights of vulnerable adults in her home.
• Failed to ensure two relief caregivers met the required qualifications.
• Failed to ensure that three people had completed background checks before they had unsupervised access to the residents.
• Failed to ensure that annual assessments were completed for two residents.
• Failed to ensure that appropriate professionals performed blood sugar testing.
• Failed to properly initialize medications and log dosages.
• Failed to ensure that appropriate persons filled medication organizers.
• Failed to ensure medications were locked and inaccessible to residents.
• Failed to ensure residents were cared for in a manner that maintained their quality of life.
• Failed to ensure the water supply was safe to drink.

The provider has the right to contest the stop placement of admissions and revocation of license by requesting an administrative hearing within 28 days of state notification. While the home is in stop placement, the licensee may not admit new residents or re-admit current residents without department permission. Current residents may stay in the home pending resolution of the appeal process. If residents choose to remain, DSHS continues to monitor the home for resident safety.
