OLYMPIA -- The state Department of Social and Health Services (DSHS) has issued a stop placement of residents and revoked the license of A&G adult family home, privately operated at 22521 - 45th Avenue SE, Bothell.
The home is licensed and owned by Olga Berjanskaia.
The state took the action after finding numerous violations of regulations that affected the safety and well-being of residents. The violations include but are not limited to:
- The adult family home's provider did not understand her responsibility to meet the needs of vulnerable adults.
- The home did not ensure two caregivers had knowledge of resident's negotiated care plans and did not ensure on-duty caregivers could communicate with one resident in her primary language.
- The home allowed two caregivers to have unsupervised access to residents, before receiving successful background inquiry results.
- The home did not keep resident medication records confidential.
- The home did not ensure one resident received necessary care and services identified in her negotiated care plan and did not ensure two residents were helped to reach their highest level of well-being.
- The home did not ensure a resident's medication was administered by a nurse-delegated caregiver.
- The home did not keep an up-to-date medication log hat showed dosages and times medication was to be administered.
- The home did not ensure all prescribed medications were kept in locked storage.
The provider has the right to contest the stop placement and revocation by requesting an administrative hearing within 28 days of state notification. The home is also prohibited from admitting any new residents pending appeal. While the home is in stop placement, licensee may not admit new residents or re-admit current residents without department permission. Residents may remain in the adult family home pending resolution of the appeal process. If residents choose to remain, DSHS continues to monitor the home for resident safety.
