OLYMPIA -- The Department of Social and Health Services (DSHS) has summary suspended the license of Morrow Manor, a privately operated adult family home at 311 Stamper Road, Elma.

The adult family home is licensed to Phyllis Stephenson.

The state took the action after finding numerous violations of regulations that affected the safety and well-being of residents. The violations include but are not limited to:

• The provider lacked understanding and ability to meet the emotional and physical care needs of three vulnerable adults.
• The provider failed to provide necessary care and services to three residents to promote safety and quality of life.
• The provider neglected and mentally abused one resident.
• The provider failed to ensure a thorough assessment of care needs and a corresponding preliminary service plan was completed for one resident before admitting him to the adult family home.

The provider has the right to contest the summary suspension and revocation by requesting an administrative hearing within 28 days of state notification. The home is also prohibited from admitting any new residents pending any appeal.
