OLYMPIA -- The state Department of Social and Health Services (DSHS) has summary suspended the license of Bingham Pond adult family home, a privately operated adult family home at 28427 - 37th Avenue South, Auburn.

The adult family home is licensed to Dillard Mailey.

The state took the action after finding numerous violations of regulations that affected the safety and well-being of residents. The violations include but are not limited to:

• The licensee failed to ensure one resident with recent hospitalizations received the medications ordered at discharge.
• The licensee failed to ensure that services were delivered in a manner that protected six residents safety.
• The licensee failed to ensure one resident in need of medical evaluation was seen by a physician in a timely manner.
• The licensee failed to ensure the AFH had sufficient staff that met minimum requirements to meet the care needs of five residents.
• The licensee failed to protect two residents' records against loss, destruction, failed to ensure one resident's record included a current medical history and admission/discharge information.
• The provider failed to maintain the home in a safe, clean and homelike manner.
• The licensee failed to update the negotiated care plan for one resident who sustained a significant change of condition.
• The licensee retained one resident whose mobility level exceeded those allowed to reside in the AFH to evacuate safely.
• The licensee failed to ensure a fire extinguisher was located on each level of the AFH.

The provider has the right to contest the summary suspension and revocation by requesting an administrative hearing within 28 days of state notification. The home is also prohibited from admitting any new residents pending any appeal.
