OLYMPIA -- The Department of Social and Health Services has suspended and revoked the license of Divine Health Systems LLC, a privately operated adult family home at 30612 - 11th Avenue South in Federal Way.

The adult family home is licensed to Serah Kamau.

The state took the action after finding violations of regulations that affected the Departments ability to determine the safety and well-being of residents. The violations include but are not limited to:

• The licensee failed to ensure the Department could make unannounced licensing inspections. Failure to allow the Department into the home to inspect placed current or potential residents at risk for living in an unsafe environment, as the Department was unable to verify the home met minimum licensing requirements.

The provider has the right to contest the summary suspension and revocation by requesting an administrative hearing within 28 days of state notification. The home is also prohibited from admitting any new residents pending any appeal.
