OLYMPIA -- The state Department of Social and Health Services (DSHS) has revoked the license of Narrows View Manor, a privately operated adult family home at 1202 Fernside Drive in Tacoma.
The adult family home is licensed to Arlie and Charlene Leno.
The state determined that the licensee failed to demonstrate the understanding and ability to provide for the health, safety and well-being of the vulnerable adults in the home. The violations include but are not limited to:
- The licensee failed to put appropriate, effective measures in place for one resident with a known history of wandering away from the home.
- The licensee failed to ensure the medications one resident received were the correct dosages ordered by her physician.
- The licensee failed to obtain nurse delegation (a registered nurse assesses the resident and teaches and supervises a person who is not a nurse) for administration of an "as needed" medication with an adjustable dosage for one resident.
- In one resident's medication orders, the licensee failed to ensure changes were written correctly on the medication log and that the adult family home received written verification of the change.
- The licensee failed to ensure all exit doors could be easily opened without a key or any special knowledge or effort.
The provider has the right to contest the summary stop placement, revocation and continued conditions by requesting an administrative hearing within 28 days of state notification. The home is also prohibited from admitting any new residents pending any appeal.
