SSI Facilitation - Home and Community Services (HCS)

Purpose: This section contains information about GAU eligibility for clients receiving HCS services, facilitation services for clients receiving HCS services, and coordination between HCS and the CSO. Home and Community Services is a division of Aging and Disability Services Administration (ADSA). This section contains information about: • GAU eligibility for clients receiving HCS services • Facilitation services for clients receiving HCS services • Coordination between HCS and the CSO

GAU Eligibility for Clients Receiving HCS Services
  1. Clients are eligible for GAU with a waiver of medical documentation:

    1. While receiving services from ADSA through an HCS office; and

    2. For 90 days following release from a medical institution where they received long-term care services from AASA.

  2. GAU clients receiving services from HCS do not have an incapacity review cycle.


Facilitation Services for Clients Receiving HCS Services
  1. When HCS refers a GAU client to the CSO for facilitation services, SSI Facilitators in the CSO provide facilitation services unless there is a local office agreement for a different procedure.

  2. The referral packet from HCS to the CSO should contain:

    1. Copy of the completed HCS Comprehensive Assessment;

    2. A completed DSHS 14-050, Statement of Health, Education and Employment;

    3. A signed SSA-827, Authorization for Source to Release Information to the Social Security Administration for each identified source of medical evidence and two extras,

    4. A signed DSHS 14-012, Authorization to Release Information,

    5. Copy of any medical documentation, and

    6. Information about any need for retroactive Medicaid coverage.

  3. The CSO sends the GAX decision to HCS.


Coordination between HCS and the CSO

HCS and CSO staff work closely together to coordinate the exchange of the following information:

  1. Change of address,

  2. SDX or BENDEX update,

  3. ACES alerts,

  4. SSI/SSDI approval or denial,

  5. Change in eligibility,

  6. New medical information,

  7. Copy of any new comprehensive assessment,

  8. Change in client advocate information,

  9. Updated personal observations or case notes, and

  10. Death of the client.
