What forms can be printed from ACES or aces.online?

 

·         An Application for Benefits (AFB) can be printed from ACES.

 

·         A Request for Benefits (RFB) and the Equal Access Plan can be printed from aces.online.

 

·         Several DSHS forms can be printed as individual attachments or included in a letter as an attachment.  See Letters chapter.

 

How do I print a form from ACES or aces.online?

 

·         To print a form from ACES or aces.online, you must have a printer defined in ACES.

 

·         A default printer can be assigned by having your ACES Security Monitor submit a security request through aces.online.

 

·         If you want your printed ACES form to go to a different printer, or if no Printer ID displays on the WMEN screen, enter the [Printer ID] in the Printer ID field on the WMEN screen.

 

·         If you want your printed aces.online form to go to a different printer take the following steps:

 

1.    Select Change Printer on the Letters Summary page.

 

2.    In the pop-up box, enter the new Printer ID in the Change to: field.

 

3.    Select if this is a permanent change (Yes / No). 

 

4.    Press Update Printer to complete the action.

 

·         If the head of household's primary language code is English, or one of the non-supported languages, the form will print in English.

 

·         If the head of household's primary language code is one of the supported languages, it is printed in the primary language as well as English.

 

Where do I find DSHS forms online?

 

·         To view DSHS forms, go to http://www.dshs.wa.gov/forms/eforms.shtml.

 

How do I print an Application for Benefits (AFB)?

 

·         The AFB can be printed when the client has not completed and signed a pre-printed application form.

 

·         The AFB can be requested only when in Option R – Interim/Hist Change or Option O – Interview.

 

·         To print the AFB, take the following steps:

 

1.    From the DONE screen, enter the client pointer in the upper right hand corner next to the 4 letter screen name.

 

2.    Press <F20> and Edit 0476 – PRINT TRANSMITTED SUCCESSFULLY displays at the bottom of the screen.

 

·         After printing the AFB, review it with the client to confirm the data is accurate before asking the client to sign it. See EAZ Manual – WAC 388-406-0010 How do I apply for benefits?

 

How do I print a Request for Benefits (RFB)?

 

·         The RFB can be printed when the client has not completed and signed a pre-printed request for assistance (sometimes referred to as Part 1 of the Application Form – DSHS 14-001X).

 

·         To print a RFB, take the following steps:

 

1.    When Screening a Client, from the Commit Status page, click on the Print RFB link.

 

2.    On the RFB, click the Print button located at the top or bottom of the page.

 

3.    When the RFB has printed, click the Next button to return to the Commit Status page.

 

4.    If the screening is complete, press the Exit Screening button or to continue screening, click the Screen New Application button.

 

How do I print an Equal Access Plan?

 

·         To print an Equal Access Plan, take the following steps:

 

1.    From the Client >> Equal Access page in aces.online, click the printable version link at the bottom of the screen.

 

OR

 

2.    When Screening a Client, from the Commit Status page, click on the Print Equal Access Plan.

 

3.    Click the Print Button located at the top or bottom of the page.
