TARGET and Treatment Analyzer Registrations

DASA processes multiple Agency Registration forms each day.  The registration process goes much quicker and is less stressful for all those involved when the form submitted is complete and accurate.  Some of the more common errors which cause delays and/or rejections are:

• Outdated Agency Registration form, the current version is on our Web site dated 6/3/05

• No User Signature

• No Agency Director Signature

• Use one form for each employee

• Use correct Fax number (360) 407-1044

• For SPAR or ATR, the County Coordinator must fill out and sign section 3

Please read and follow the instructions on page 2 of the Agency Registration form.  If you need additional help, contact the helpdesk at 1-888-461-8898.  Attention to detail in filling out the Agency Registration forms will enhance the process efficiency and responsiveness to user needs.

Downtime

The TARGET system is typically not available the last weekend of each month from 5:00 p.m. Friday, through 8:00 a.m. Monday. The dates for this month are: October 27th thru the 30th.
