Case Management Services (DDD)

What the program covers: DDD case managers determine eligibility for services, assess the needs of individuals and their families, and connect eligible persons to available services to meet these needs. Case managers coordinate planning and development of resources, authorize payment, monitor and review service delivery, provide information about available services, and refer persons to other sources of support. Case managers also assist consumers and their families with crisis intervention.

Who is eligible: DDD eligibility requires that a person meet all of the following conditions as defined in WAC 388-823:

A developmental disability is defined in RCW 71A.10.020(3) and must meet all of the following requirements. The developmental disability must currently:
  1. Be attributable to mental retardation, cerebral palsy, epilepsy, autism, or another neurological or other condition found by DDD to be closely related to mental retardation or requiring treatment similar to that required for individuals with mental retardation;
  2. Originate prior to age eighteen;
  3. Be expected to continue indefinitely; and
  4. Result in substantial limitations to an individual's adaptive functioning.
DDD eligibility may be time limited and subject to review. Contact your nearest DDD office for an intake appointment. The case manager will assist you in obtaining documentation needed to determine whether you are eligible for services from DDD.

Income and Resource Limits: None

Program Requirements: The person must meet DDD eligibility criteria to receive case management services. If the person is eligible, the case manager will conduct a needs assessment and develop an individual service plan.
