Rehabilitation Plan

Now that you have completed the vocational assessment and have made a decision about your employment goal,  your DVR counselor will assist you to develop a written plan of services. The "Rehabilitation Plan" outlines the decisions you and your DVR counselor have made regarding the specific services necessary for you to reach your employment goal.

Specifically, the Rehabilitation Plan will include:

• Your vocational goal
• The steps necessary to reach your goal
• Services needed to become employed
• Costs for each service
• Who will pay for services
• Terms of agreement (your responsibilities and DVR’s responsibilities)
• Timelines

The Rehabilitation Plan is the agreement made between you and the DVR counselor, and becomes effective when signed by all parties. We consider the Rehabilitation Plan a commitment on our part to fulfill the terms of the agreement to the best of our ability. You also need to understand the importance of the Rehabilitation Plan and be fully committed to carrying it out when you sign it. This is the document that will guide you toward achieving your employment goal.

The Rehabilitation Plan can be modified to reflect a change in circumstances. Major changes would require agreement and signatures of both you and your DVR counselor.
