Eligibility

You are eligible for DVR services if you:

1) Have a physical or mental disability that results in a substantial barrier to employment; AND
2) You require vocational rehabilitation services to get or keep a job.

You will work with DVR staff to collect information about your disabling condition(s), barriers to employment and your ability to benefit from vocational rehabilitation services. DVR must also collect documentation that proves you can legally work in the United States. DVR must have copies of the documents listed below before we can determine your eligibility for services.

Identity (provide a copy of one of the documents listed)

1. Valid state-issued driver’s license, with photograph; or
2. Valid state-issued identification card with photograph, or with name, birth date, gender, height, color of eyes, and address;
3. Valid U.S. military card, with photograph; or
4. A current passport.

Employability (provide a copy of one of the documents listed)

1. Valid Social Security card; or
2. Current re-entry permit (INS Form I-327); or
3. Current refugee travel document (INS I-571); or
4. Certificate of birth (Form FS-545); or
5. Certificate of birth abroad (Form DS-1350); or
6. Original or certified copy of birth certificate issued by a state, or recognized sub-division, establishing birth in a state; or
7. Native American tribal document; or
8. Valid U.S. passport.

It is helpful if you bring copies of your medical reports and other documents to your appointment.
